Attention Parents of Students Entering Kindergarten

The following items are needed to set up your child’s student file:

Please email these four documents to Mrs. Gorney at

  1. Birth Certificate

  2. Baptism Certificate

  3. Immunization record **

  4. A physician completed SCHOOL ENTRY HEALTH CHECKUP form (Note: due to COVID-19, the deadline to submit this form to STA is extended to May 2021)


**California state law requires that all incoming kindergarten students have received the following immunizations prior to the start of school:


Please help us…send in your immunization record today. If you are notified of missing immunizations, please schedule an appointment as soon as possible. Your neighborhood Walgreens, Rite Aid or CVS can also administer immunizations. Thank you for your help!
